The Franklin VNA & Hospice is pleased to offer in-home electronic telemonitoring using the HomMed Telemonitoring® System.  The home monitoring system performs daily home assessments of preprogramed information.  We can monitor our patients 7 days a week and make our visits as patients' needs dictate.  The device uses verbal prompts to walk the patient through collecting their vital signs every day at a set time.  We are also able to program several individualized yes / no questions to assist in areas such as medication compliance, respiratory symptoms, and pain.  The results are automatically transmitted securely to the Franklin VNA & Hospice over the phone line via an 800 number, or via wireless pager service where they are reviewed by a registered nurse.  This process takes about 3 minutes to complete.  There is no additional charge for telemonitoring of patients who meet the skill criteria.

This system allows us to be more preventive and proactive in our care, keeping patients out of the Emergency Room and in their doctor's offices for scheduled visits. It also helps to improve the communication betweeb our agency and the doctors office and gives the patient/family peace of mind.
