Talk to your Physician

Most referrals for Home Health Services are made by doctors and / or their staff.  Physician referrals can be made easily using our secure fax line at (603)934-1234 or by calling (603)934-3454 and asking for the Intake nurse. 

Concerned Family or Friends 

Referrals for Home Care or Hospice services may be made by anyone.  Although most referrals are made by physician offices or hospitals.  Families, concerned friends or neighbors can request that an individual be evaluated for needed assistance by calling (603)934-3454.  The agency clinician will talk with the person's doctor to determine whether that person is eligible for Home Care services under Medicare guidelines.  Often friends or neighbors are the first to see the changes in one another and become concerned for safety issues.
